Is this confirmed to work with WP 3.0?

I&#039;ve installed the php code on my static page and haven&#039;t done much else.  It shows the default Post Title *, Post URL, and Description * fields. When I fill out the form it tells me all &quot;You didn&#039;t fill in all required fields.&quot;

Am I missing something simple?
